Better Experience With C Section Second Time Around Panic Attack – One of the first questions many women who are pregnant for the second time ask their doctor is whether they can have a vaginal birth if they’ve had a previous C-section.

The answer, say two Penn State health experts, is that it depends on their experience with their first child.

“I always wonder why they did a C-Section,” said Dr. Christina DeAngelis, an OB/GYN at Milton S. Hershey Medical Center. “Were there complications? What are the risk factors for not being a candidate for labor after cesarean?”

A Week By Week C Section Recovery Timeline

If the patient had a cesarean delivery due to a low-risk factor, such as a breech delivery, there’s every reason to believe she might have a vaginal birth later, DeAngelis said. But if a patient is having a difficult labor, has medical complications such as diabetes or high blood pressure, or has given birth to a large baby, vaginal birth can be difficult.

Nationally, 30 percent of first-time mothers have a C-section, and Penn State Health patients fall into that category. Dr. Jamie Pauley, an OB/GYN at Hershey Medical Center, said that while many women who have a C-section want to talk about their options right away, if they don’t discuss it at the first visit, she does It’s important to start thinking about the possibilities, she said, and she also wants women to know that their thoughts on the issue matter.

Some women have misconceptions about second births. They may hear from their mothers and other relatives who gave birth decades ago that once a woman has had a C-section, she cannot have a vaginal birth. Pauley says he has patients who are shocked to learn it isn’t.

What patients should remember is that the best way to give birth is to have ongoing conversations throughout the pregnancy, both doctors say.

“It’s an important conversation to have all the time,” Pauley said. “Women should be part of their health care experience and feel like they are part of the decision-making.”

It’s also important that women know they can change their minds about how they want their pregnancy to develop, DeAngelis said. About 20 percent of her patients decide to have a C-section or vaginal delivery early in pregnancy, she said. Some others struggle with the decision within nine months.

One tool both doctors use when discussing their options with a patient is a calculator that predicts whether a vaginal birth is likely to be successful based on a number of factors. This data is added to the mix when considering what happens on the day of delivery.

A general rule of thumb, DeAngelis said, is that if a woman who has had a previous C-section has to go into labor spontaneously with her second child, the chance of breech birth increases. If she is past her due date and her body is not showing signs of readiness for labor, a C-section may be recommended.

Other important things to remember: Home birth is not recommended, especially for women who have had a C-section and want to have a vaginal birth, Pauley said. Even with the best plans and a healthy pregnancy, something unexpected can happen during birth, and patients need to be in a hospital where a C-section can be performed quickly and there is immediate access to emergency resources.

Another factor is the number of children a woman wants. If a large family is being considered, DeAngelis said, miscarriages should be considered. The risk of complications increases with multiple C-sections.

One way to help women is to keep the incidence of first cesarean births as low as possible, which Penn State Health doctors believe is important.

Ultimately, though, Pauley says there’s one birth variable we can’t always control: “Ultimately, the baby is responsible.”

C-sections account for one-third of births in the United States—and while C-sections are generally very safe (and in some cases life-saving) for mothers and babies, there are risks and potential complications that should be avoided. be aware Because of these risks, your doctor or midwife will probably discourage you from having an elective C-section. If your provider offers a scheduled C-section, we recommend asking about minor C-section changes that can help make your C-section feel more intimate and less clinical. Also, ask if your hospital has an ERAS protocol, which can help you recover faster after surgery.

A cesarean section, or C-section, is surgery to deliver a baby through incisions in the mother’s abdomen and uterus. In some cases, a C-Section is pre-planned. In others, surgery is needed because of an unexpected problem. If you or your baby is in imminent danger, you will have an emergency C-section.

In the mid-1990s, C-section rates began to rise significantly, with doctors trying to reduce the use of unnecessary C-sections, but sometimes C-sections to protect the health of the mother or her baby. is necessary

How Many C Sections Can You Have?

Expect a C-Section to last between 45 minutes and an hour and a half. A baby’s extraction is usually very quick, from 1 to 15 minutes. Breastfeeding with a closed uterus and stomach takes longer.

Your partner is usually with you during a C-Section. He will be asked to wear an operating room gown and sit next to your head.

In some cases, a C-section is safer for you and your baby than a vaginal birth. For example, your provider may recommend a planned C-section if:

Unless there is a medical need before the baby is born, your midwife will not have your operation for more than 39 weeks.

The Truth About C Sections Revealed

You may need an unplanned or emergency C-section if problems arise that make continued labor dangerous for you or your baby, such as:

Elective C-sections are C-sections that are not medically necessary and are advised by most healthcare providers. This is because having an unnecessary C-section is more dangerous for you than not having a vaginal birth. Also, having an elective C-section increases your chances of needing a C-section with a future delivery.

Medical organizations, including the American College of Obstetricians and Gynecologists (ACOG), recommend planning vaginal birth whenever possible. Unless the health of the mother or baby is at risk, ACOG says the risks associated with a C-section usually outweigh the short-term benefits.

Elective C-sections or “demand mothers” are less common. Reliable figures are hard to come by, but most experts estimate that fewer than 3 in 100 women request a C-section for their first birth. Some fear the pain of childbirth, others worry about vaginal birth complications, such as rupture, incontinence and sexual dysfunction, and some find that planning a C-section can be unpredictable. Giving birth is easier said than done.

If you have any of these concerns, talk to your provider. But because of the risks — including a difficult recovery and a higher chance of complications — be prepared for your doctor or midwife to discourage you from having an elective C-section. However, your provider should be willing to have an honest conversation about your concerns and needs, bringing them up early in your pregnancy to allow time for ongoing discussions.

“Soft C-section” is the name of the changes your labor team can make to make the C-section less clinical and more intimate. Depending on the terms of your submission, this may or may not be the case.

While there isn’t much research on the effects of gentle C-sections, one study found that women had significantly increased satisfaction with their birth experience when gentle C-section techniques were used. Immediate skin-to-skin contact also helps regulate the baby’s body temperature and heart rate.

The Enhanced Recovery After Surgery (ERAS) protocol is designed to help patients recover faster from surgery with fewer complications. Not all hospitals use ERAS and different hospitals implement it in different ways. For moms who have had a C-Section, ERAS often means you’ll receive medication by mouth before surgery and long-term pain relief under spinal anesthesia so you have less need for strong narcotics after surgery. You may be prescribed non-drug methods of pain control, such as a lidocaine patch over the incision and a prescribed dose of ibuprofen and acetaminophen around the clock. You will be encouraged to get out of bed and try to walk within six to eight hours of giving birth.

C Section Checklist

In general, cesarean sections are very safe. But a C-section is major abdominal surgery, so it’s more likely to cause complications for you and your baby than a vaginal birth and have implications for future pregnancies.
